MANAGEMENT OF CONGENITAL PSEUDARTHROSIS OF THE TIBIA USING THE ILIZAROV TECHNIQUE: CLINICAL AND RADIOLOGICAL OUTCOMES

Congenital pseudarthrosis of the tibia (CPT) is a rare and complex orthopedic disorder characterized by persistent nonunion, deformities, and limb length discrepancies. This retrospective study evaluated 17 patients treated with the Ilizarov technique combined with pseudarthrosis excision and fixation at Sree Balaji Medical College & Hospital, Chennai, with a minimum follow-up of two years. Functional outcomes were assessed using the American Orthopaedic Foot and Ankle Society (AOFAS) score, which showed significant improvement postoperatively. Radiological and clinical results demonstrated effective bone union and correction of deformities in the majority of cases. However, long-term complications including limb length discrepancies, ankle valgus, and malalignment remained challenges requiring ongoing management. Careful surgical planning, maintaining a wide cross-sectional area at the docking site, and transitioning to intramedullary fixation after frame removal were recommended to minimize refracture and joint stiffness. Patient satisfaction was high when stable union and functional weight-bearing were achieved, despite residual deformities. This study underscores the importance of individualized treatment and long-term follow-up in managing CPT to improve functional outcomes and quality of life
